Manager should communicate with team member so the issuing of a warning letter isn't a surprise

The reason behind issuing a warning letter is to formalize and document the discussion and agreed course of
 correction to take

PROCESS FOR MANAGERS
write up the issue on warning letter and send to HR first before giving discussion
once HR confirms they will reply back
arrange time to have meeting with employee ,give employee notice that you wish to discuss the issue

at meeting explain the issue to employee and explain required course of correction

employee/manger to sign off on form
copy to employee
copy manager passes to hr to keep with employee file

the key is that the employee clearly understands the expectations and the issues that their actions cause
